A cosmetic surgeon is an expert in managing scars. A great deal of education and training goes into giving patients the best scars possible, since this is a big part of any surgery. It is important to make a distinction between fresh and old scars because the handling of each is different. Many factors affect the final scar outcome, such as the location and size of the scar, how the scar was obtained, the quality and age of the skin, the color of the skin, the amount of sun exposure to the scar, and other factors. People have a tendency sometimes to call any unsightly scar a keloid scar. A scar can be a poorly healed, exaggerated, or an “ugly” scar, but is not necessarily a keloid.
A keloid is a very specific scar and not a “catch-all” term. When the skin is cut, the body lays down scar tissue to heal the opening and close it. Once the opening is filled with scar tissue up to the level of the skin, the body stops building scar tissue. In one specific abnormal circumstance, the body does not know when to stop, and keeps building scar tissue. The result is an overabundance of scar tissue above the skin surface. This is a keloid scar. There are multiple surgical and non-surgical ways of treating keloids, from direct excision, to silicone pressure sheets, to steroid injections, or low-dose radiation therapy. Sometimes we use a combination of two procedures to obtain the best results.
When a patient is inquiring about a cosmetic procedure, the physician will try to clearly describe how the resulting scar would appear. An upper eyelid lift scar heals very nicely because the skin is very thin and the tension is minimal. In contrast, a tummy tuck scar is in an area where the skin is thick and the tension is greater, so the results are not the same. When someone comes to our institute complaining of an old scar that they would like to improve, they are handled on a case-by-case basis. The scar will be assessed and the patient will receive different treatment options that may include excising out that scar to give the patient a “nicer” scar, laser treatment, microneedling, treating it with the scar tape and cream program, massaging it, treating it with silicone sheeting, and other modalities.
We strive to achieve patient satisfaction and provide the latest and greatest treatment options while managing patient expectations. Scars can be improved but never erased.

We believe a well-prepared patient leads to a successful surgery and recovery. Whether you are flying in internationally or a local, our staff strives to give you a great experience. During your initial call to our office, one of our patient care coordinators will discuss the different options available to improve the areas of your concern. A consultation is scheduled with the doctor who best fits your personal situation. The doctor will review your medical history, spend the appropriate time to review all surgical and non-surgical options, and discuss whether surgery can be performed in-office under local sedation or at the hospital under general anesthesia. In sharing our before and after photos, you get a better sense in the quality of surgery our doctors offer and what outcome you can expect to achieve. We will review pre- and post-operative care so you can be prepared for what your role is in surgery. You will be asked to stop smoking two weeks prior to and post-surgery. Next, we see you for a pre-op appointment two weeks prior to surgery to discuss any questions you may have and review surgery details, ensuring everyone is on the same page. You will then receive a courtesy call the evening before surgery to review instructions and any questions you may have.
THE SCAR REVISION PROCEDURE
The procedure takes about 30 – 60 minutes, depending on the size and location of the scar. For very large scars, the revision may be performed under general anesthesia or heavy sedation in the hospital operating room, and not in the office operating room. If a patient is a good candidate for the procedure, an anesthetic is applied. The scar is excised and closed with a meticulous cosmetic closure. Sutures used may either be absorbable sutures, or may need to be removed, depending on the scar. Your options will be discussed before the procedure.

To promote healing and minimize complications, you will be seen in a series of post-op appointments, including the day after surgery. These appointments will continue throughout the first year after the procedure to make sure you are healing as planned. For your added convenience, you also have direct access to Dr. Bitar’s and Dr. Lickstein’s cell phone numbers if you need to reach your physician after hours. The final post-op visit, one year after surgery, is one of our favorites since the results are fully apparent and we love seeing our patients happy. You are always welcome to come back and see us, even years after your surgery.
